Lovelace Health Plan’s Disease Management Department offers two programs that assist members with managing a chronic condition or quitting smoking. The LIFELONG Steps program identifies members with certain conditions. Registered nurses work with the higher risk populations to prevent or delay complications and improve the quality of life of these members. The diseases currently being managed include:

·        Asthma (both pediatric and adult populations)

·        Chronic Obstructive Pulmonary Disease (COPD)

·        Type 2 Diabetes (adults)

·        Heart Failure

·        High Cholesterol (secondary prevention of another cardiac event)

 

An additional LIFELONG Steps program offered to all LHP members is the Healthy Weight Initiative. This is a self-referral program that assists members wishing to attain or maintain a healthier weight.

 

The second Disease Management program, Stop Tobacco for Optimal Prevention (S.T.O.P.) is another self-referral program offered to all LHP members. This program helps members with quitting smoking. The S.T.O.P. program also has a pharmaceutical component.

 

All the above programs are telephonically based. Outbound calls made by trained registered nurses are a key intervention that provides members with education, guidance, tracking of recommended care, and use of motivational interviewing techniques to help with positive behavior changes. Other interventions to members not currently being telemanaged may include reminder notices for services due or educational mailings.

 

Lovelace Health Plan and Lovelace Sandia Health System have other programs aimed at improving the health of its members. These include:

·        Health Risk Assessments (online with immediate feedback)

·        Body Balance (online prevention and wellness education and services)

·        BabyLove (Assist members through their pregnancy)

·        Health Fairs

·        Health Matters (Community-based health education classes)
